Aplus 50 unit Novorapid/50 cc NaCl 0.9% (setara 1 unit/1 cc), drip:
KGD > 400 : 6 cc/jam
KGD 350 - 400 : 3.5 cc/jam
KGD 300 - 350 : 3.0 cc/jam
KGD 250 - 300 : 2.5 cc/jam
KGD 200 - 250 : 2 cc/jam
KGD 150 - 200 : 1 cc/jam
KGD 100 - 150 : 0.5 cc/jam, stop drip, Beri SC Lantus dosis basal
KGD < 100 : Stop drip, bolus Dex 40%. (1 flacon estimasi naik 50)

Note:
Kebutuhan insulin = 0.6 Unit/kgBB/hari
Dibagi ke dalam: Prandial 60% (dibagi 3 untuk P-S-M) dan Basal 40% (untuk M sebelum tidur)

Penyesuaian dosis insulin basal:
< 70       : Down dosis 2 unit
70 - 130 : Pertahankan dosis
> 130     : Up dosis 2 unit
> 180     : Up dosis 4 unit
